We take the referrals that do not fit a standard box.
When a plan is written but nothing is moving, the person waits. Case managers are left searching for a provider who answers the phone, documents properly, and takes the referral that doesn't fit neatly anywhere.
We were built for exactly those referrals.
WHAT WORKING WITH US LOOKS LIKE
A plan is only as good as the provider who carries it out. You get a partner who communicates early, documents properly, and never lets a problem arrive as a surprise.
Send a PAR and client information and we respond the same business day. We are glad to talk a referral through before you commit to it — and we will tell you honestly when we are not the right fit.
You work directly with our Administrator, a Board Certified Case Manager, not a queue.
COORDINATION IS PART OF THE JOB
Designing independence for one person usually means coordinating across several people who have never spoken to each other — family, therapists, teachers, employers, medical providers, support teams. We take that on rather than assuming someone else will.
DOCUMENTATION THAT HOLDS UP
Our policies, forms, and staff training were built to the Colorado HCBS rules and CDPHE licensure standards from the ground up. Incident reporting, rights modifications, supervisory visits, and service documentation are survey-ready — so authorizations stay clean and your file stays defensible.
Our initial state survey was completed with zero deficiencies.
WE PRESUME COMPETENCE, AND WE MEAN IT OPERATIONALLY
Presuming competence does not mean expecting everyone to need the same things. It means we start without assumptions and design around the person, rather than fitting them to what we already had staffed and scheduled.
That shows up in the plan, in how choices get documented, and in whether a rights modification is genuinely necessary or just convenient.
