Services are designed to support participants in remaining as independent as possible within their homes and communities while promoting health, safety, dignity, and self-sufficiency.
Staff provide support with routine household activities that participants are unable to complete independently due to physical, cognitive, developmental, or functional limitations. Services are individualized based on the participant’s assessed needs, personal goals, and person-centered service plan and are intended to promote skill development and increased independence whenever possible.
Supports may include meal planning and preparation, laundry and linen care, dishwashing, household organization, light housekeeping, grocery shopping assistance, trash removal, and other household tasks necessary to maintain a clean, healthy, and safe living environment.
Services are provided within the participant’s residence and may include support related to household management activities necessary for successful community living as identified in the participant’s service plan.
